This form is to collect information needed to provision an account for your extramural research program. To proceed, you have to have the following steps completed before we can start with the provisioning process:
• Your Extramural Institution has been enrolled in the STRIDES Initiative (Please visit the Institution Enrollment form to complete this requirement)[Opens a new window.]
• You have the approval from your Program Officer to request this account provisioning

Based on this provided information, we will start the account provisioning process for your research program in the NIH STRIDES Initiative.

Summary and Justification (Brief description of the research problem clearly identifying the direct relevance of the project to biomedical research)

Keywords (List of terms describing scientific area(s), technologies, and/or conditions relevant to research project)

Scientific Institution (Examples: Harvard Medical School, Mayo Clinic, University of Chicago, etc.)

Administering NIH Institute or Center (Derived from Notice of Award)

NIH Funded Project Award/Grant # (Derived from NIH Notice of Award, uniquely identifies NIH-funded research projects)

NIH Program Official Name (Derived from Notice of Award)

NIH Program Official Email (Derived from Notice of Award)

Technical or Additional POC (Technical and Scientific POCs can be the same. Additional individuals who will receive future communications from STRIDES (including monthly billing reports) )

Technical or Additional POC Email

Job Title (Job title or affiliation with the research program)

Scientific POC (Principal Investigator or other awardee who has overall responsibility for scientific direction, responsible for setting (or delegating) security policies and financial oversight of cloud resources)
